    Our History

     

    Founded in 2017, Assurance Social Services and Case Management was established with a clear mission: to bridge gaps in care and ensure individuals and families receive the guidance, advocacy, and coordination they need to navigate complex health and social service systems. From its inception, the organization has focused on delivering compassionate, client-centered support while promoting dignity, independence, and improved outcomes.

    In its early years, Assurance recognized a growing need for comprehensive case management services, particularly for individuals facing medical, social, and financial challenges. By bringing together experienced professionals with backgrounds in healthcare, social work, and community advocacy, the organization built a multidisciplinary approach that emphasizes collaboration with hospitals, families, and community providers.

    Over time, Assurance expanded its scope to meet evolving community needs. The organization now offers a full continuum of support services, including:

    • Care Management — Coordinating medical, behavioral, and social supports to create personalized care plans.
       
    • Complex Discharge Planning — Working closely with hospitals and rehabilitation facilities to ensure safe, smooth transitions from acute care to home or appropriate placements.
